The mission of the Vannie E. Cook Jr. Children’s Cancer and Hematology Clinic is to offer comprehensive care and treatment to children of South Texas who suffer from cancer and blood disorders. The Vannie E. Cook Jr. Children’s Cancer and Hematology Clinic, a joint effort of the Vannie E. Cook Jr. Cancer Foundation, Baylor College of Medicine and Texas Children's Hospital is the region’s first state-of-the-art pediatric hematology/oncology clinic. In addition to offering diagnostic and treatment services, the clinic provides cancer genetic counseling to individuals and families concerned about their risk of cancer, offers the services of a long term survivor program which follows childhood cancer survivors through adulthood, and conducts childhood cancer epidemiology research through a joint program with M.D. Anderson Cancer Center.
How to Make a Referral:
During business hours - Monday through Friday 8 a.m.-5 p.m.:
Referring physicians may call 956-661-9840.
Please print and complete the following form(s):
- Patient Referral Form (PDF)
- PCCM Referral Form (PDF) (in addition to the Patient Referral Form if patient is enrolled in the Medicaid PCCM program)
The referral form(s) along with current labs and a copy of insurance, if applicable, should be faxed to 956-661-9841.
Clinic physicians will review the referral and assign an appointment. The referring physician’s office will be contacted with the appointment date. The referring physician can request to speak directly to the clinic physician and perform a physician to physician referral.
During non-business hours, weekends and holidays:
Referring physicians may call the Vannie E. Cook Jr. Children’s Cancer and Hematology Clinic answering service at 956-661-9840 and ask that the physician on call be paged in order to conduct a physician to physician referral.

How to Make a First Appointment:
A referral from your physician will need to be obtained in order to make an appointment at the Vannie E. Cook Jr. Children's Cancer Center and Hematology Clinic.
Why do I need a referral?
We ask that your physician give us a call before your first appointment is made. During this phone call, your child's case is discussed and it is decided how soon the patient should be scheduled for an appointment at the Vannie E. Cook Jr. Children's Cancer and Hematology Clinic. This conversation also helps to determine what laboratory tests will be necessary at the time of the visit.
What to Bring to the First Appointment:
Be sure to bring as many of the following items as are available to the first appointment:
- Immunization and medical records of the patient
- If x-rays and/or scans have been obtained, please bring a copy preferably on a CD (digital copy) if available
- If available, please bring pathology slides and paraffin blocks
- Insurance cards for proof of insurance
- The Social Security Number of the insurance holder in order to verify insurance coverage
